What you'll need to complete this form

We'll need you to provide:

• the full name of the deceased person
• the date of their death

Please provide as much information as possible as most of the records need to be examined manually. If we can trace the person, we will send you a summary of the information recorded in the cemetery or crematorium book. If we can't trace the person, any fees paid can't be refunded.

Payments

Once your application has been successful, you will be asked to pay the corresponding costs. We accept card details over the phone, cheques and cash.
