If you do not have any of the above, you'll need to supply medical evidence that your walking ability is affected. You may also be required to have an eligibility assessment.

If you currently hold a driving licence, and you do not meet the required standards for driving because of your mental or physical health condition, you must inform the DVLA. We'll need proof that your licence has been surrendered, plus your supporting documentation to enable us to issue you with a bus pass.
If you’ve changed your name, you need to provide proof, such as a marriage certificate or deed poll.
If you have moved, you need to provide proof of address dated within the last 12 months.
You will not automatically receive a new bus pass by changing your details. If you would like to order a new pass with a change of name or photo, select the 'replacement' option from the dropdown menu.
