Transition means moving from childhood to becoming a young adult.
Transition planning normally starts around the age of 14.
We call this transition planning.
It explains what everyone will do to make sure you get the support you need as you grow up from a teenager into an adult.
It doesn’t replace an EHC plan.
You, your parents, carers and other people who support you will go to a meeting at your school in year 9.
At the meeting you will all talk about:
After the meeting, your school will put together the Transition Plan and send it out to everyone involved.
The plan will be updated every year until you leave school.
If we think you may need adult social care support when you’re over 18 years old, we'll start planning for that when you’re 16 years old.

You can ask for support from the Social Work team, this is called the Specialist Community Disability Team (SCDS).
Contact the Specialist Community Disability Service to get support for people with learning disabilities.
You can also ask for support from an advocate who is independent.
