Active for Life registrations

How to apply

To register for an Active for Life session or event simply download and complete the relevant Adult or Under-16’s form below and send it in an email to healthylifestylesteam@brighton-hove.gov.uk.

To avoid any delay you need to:

• tell us the specific activity you would like to attend
• answer all questions in your registration form
• ensure Under 16 forms are completed by a parent or guardian

Once we have received your application, it will be processed and you will receive either an email or a phone call, depending on what you submitted.

Attending events

Please do not attend an activity until you have received a confirmation from us.

We will reply as soon as we can although this is subject to availability. We encourage you to register as early as possible.

Completing the form

We suggest you try using Google Chrome to download the form and open it with the latest version of Microsoft Word if possible.
