Job introduction

An opportunity has arisen for a Care Manager within the enhanced duty team of the Specialist Older Mental Health Service (SOAMHS) based at East Brighton Community Mental Health Centre. This is a fulltime permanent position and will be based at East Brighton Community Mental Health Centre. There may be a requirement to work out of hours and at weekends on a rota system. The operating hours of the service are 9-7 Monday to Sunday for which enhancements will be paid.

About the role

The enhanced duty team offer intensive assessment and support to people with dementia. It is a multi-disciplinary team and is part of the wider Specialist Older Adults Mental Health Service (SOAMHS).  This service is currently under review and there may be an expectation for you to work as part of the wider SOAMHS team and there will be an opportunity to work out of care 9-5 hours.

We are looking for someone who has high professional standards with an excellent knowledge of current legislation such as the Care Act and Mental Capacity Act.

As a Care Manager you will support the delivery of statutory duties placed upon us by the Care Act. The main duties of this role will be to undertake reviews, assessments, professional interventions and other duties relating to individuals, families and carers. In order to fulfil these duties, you will be required to work collaboratively with all stakeholders to improve outcomes and promote wellbeing, and demonstrate knowledge in the areas of personalisation, mental capacity and social models of support.

The successful candidate will:

  • Be able to work successfully alongside multidisciplinary colleagues.
  • Have an understanding of the importance of information and advice in supporting people to maintain their own independence and wellbeing.

You will receive regular supervision and support and have opportunities for ongoing development and specialist training.

Additional information

Your starting salary will be pro rata if the above position is less than 37 hours or term-time only.
