Local support to be active with long-term conditions
Being active can be very beneficial if you are living with a long-term condition. It can help you feel better physically and mentally if you move more in a way that suits you.
Your GP, nurse, physio or other health care practitioner can refer you to the Freedom Leisure Health Referral Programme.
This programme offers discounted gym, swim, and classes to people with health conditions that could benefit from moving more. You will also have three 1-2-1 sessions with a qualified Exercise Referral Instructor.
