Preparing for adulthood for 16 to 25 year olds with SEND
If a young person has an Education, Health and Care plan, schools and other services involved in supporting them must help the young person to prepare for adulthood.
The young person’s aspirations and goals should be recorded in their Education, Health and Care plan and school, social care and health services should be supporting the young person to achieve their goals.
