Age UK Crisis Service- Level 2

Level 2 service offers support to older people to help increase confidence, prevent falls and maintain independent-living. This can involve a one-off visit to provide advice on preventing falls, or a number of regular visits over a period of weeks from a volunteer to help rebuild confidence and work towards self-selected goals.

Level 2 can also provide signposting to other voluntary services that can help with falls prevention and rehabilitation.

Headway

A national charity that works to improve life after brain injury. Through their network of more than 125 groups and branches across the UK, they aim to promote understanding of all aspects of brain injury to survivors, their families and carers.

Sussex Community NHS Trust - Sussex Rehabilitation Centre

A local service providing assessment, treatment and advice to people with neurological impairment, including stoke, traumatic brain injury and Multiple Sclerosis. The objectives of the service are to improve physical well being, functional status and decrease the care needs of patients.

The centre also provides a specialist OT service, where they assess and prescribe attendant propelled and electrically operated indoor and outdoor wheelchairs. Referral is usually via GP's.

Sussex Rehabilitation Centre Out-patient Service Brighton & Hove

The service provides assessment, treatment and advice to people with neurological impairment, including stoke, traumatic brain injury and Multiple Sclerosis. The objectives of the service are to improve physical wellbeing, functional status and decrease the care needs of patients.

The service is for individuals living within Brighton and Hove and some patients from East Sussex. Out-patient follow-up is also provided for patients from East and West Sussex who have been in-patients at Sussex Rehabilitation Centre (Princess Royal Hospital, Haywards Heath).
