NHS Brighton and Hove Clinical Commissioning Group (CCG)

The Clinical Commissioning Group (CCG) is led by a group of local doctors and nurses and brings together all GP practices in the city. They also have a network of patient participation groups (PPGs), which are run locally by volunteer patients and practice managers who contribute ideas and feedback suggestions directly to their practices.

Sussex Community NHS Trust - Responsive Services

Responsive Services are a multidisciplinary team who visit patients in their home, including patients who live in care homes or temporary accommodation. Responsive Services provide short term health support to help people recover and remain in their own home.

The service covers the whole of Brighton and Hove but are only available for people who are registered with a Brighton and Hove GP.

Responsive Services provide short term nursing, physiotherapy and occupational therapy assessment and support and aim to work with people to increase their independence following an episode of ill health.

Referrals are accepted from health or social care providers. The public cannot make direct referrals to Responsive services.

Referrals for Responsive services should be made via the referral management hub (RMH).
