What is a social care assessment?
An assessment is simply a way of finding out your needs and providing the kind of help available, so that you can continue to live in the community. Carers (people who look after you) are also entitled to an assessment of their needs.
How do I get help in an emergency outside of office hours?
If you have an emergency between 5pm and 8.30am Monday to Friday, or anytime during weekends or bank holidays, contact the Emergency Duty Service on (07699) 391462.
