Community Short Term Services

Community Short Term Services is a short term health and social care service for people living at home or using care beds at Craven Vale Resource Centre, Brighton, and Knoll House, Hove. These care beds are assigned to patients as an intermediate measure. The service is currently looking to increase the number of beds.
The service aims to support people discharged from hospital and prevent unnecessary admission to hospital or long stay care. It offers the opportunity for recovery and rehabilitation through a planned programme of care and treatment.
Social workers, nurses, home care support workers, physiotherapists, occupational therapists and administrators work together within Community Short Term Services.
Knoll House, a short-stay rehabilitation unit with 20 beds, is part of citywide Community Short Term Services.Health promotion activities are based at Knoll House, and a locality falls clinic is being developed. Nurses and therapists on site will offer sessions on rehabilitation, well-being, and health promotion. Aiming to prevent unnecessary admission to hospital or long-term care and also to facilitate discharge, Knoll House will help people to recover more rapidly and to regain their former independence through planned programmes of care.
The service covers people living in Brighton & Hove or who have a Brighton & Hove doctor.

Requests for service are accepted from health or social care professionals, eg community nurses, GPs, social workers, health visitors and hospital staff.
Who is eligible and what is the cost of the service?
To be eligible for this service, the person needs to:
- be considered able to benefit from this short term service and willing to participate in a rehabilitation programme
- live in the Brighton & Hove area or have a Brighton & Hove doctor
- be aged 18 years or over
The service is free up to a maximum of six weeks only.

Frequently asked questions
What are the benefits?
The service gives you the opportunity to receive specialised care and treatment outside hospital, with medical care provided by your own doctor.
Do I have to pay for anything?
The service is free for up to a maximum of six weeks. If you need ongoing social care after six weeks, you will have a financial assessment and may be required to contribute in line with the local authority charging policy. We can also refer you to community nurses, health visitors or other agencies who can offer support.
What happens if my situation changes?
If your condition changes while we are looking after you, we will contact your GP who will decide if you need to be admitted to hospital. If this is the case so, we will help arrange this.
Can I refer a relative or myself?
Not at present. We feel it is important for your doctor or consultant to see you first, so that a full medical assessment has been made to ensure that you are treated in the most appropriate way to meet your needs.
