16 Plus Support Team (providing the Leaving Care Service)
The 16 Plus Support Team works together with 16 and 17 year old young people in the care of Brighton & Hove City Council and 18 to 21 year olds who have left its care. The team includes Social Workers, Personal Advisers, a Nurse and specialist workers in employment and training and supported lodgings and housing. We advise and support young people as they get ready to leave care and help them to move on in their lives into adulthood and independent living.
If you are aged under 21 and have been looked after by Brighton & Hove City Council you are entitled to advice, support and assistance from the 16 Plus Support Team.
We can help with:
· the move from living in care to adult life and independence
· practical and emotional support
· education and training
· employment
· health and leisure
· financial advice and assistance
· family and social contact and relationships
· What to do when things don’t go to plan
We respect people’s differences and will treat young people fairly and recognise their individual requirements and additional needs. We will challenge discrimination and prejudice and will investigate cases where young people believe that this has taken place.
Young people will have their own Personal Adviser who through a reliable, supportive and trusting relationship will advise, support and assist them at least until the age of 21 years and up to 25 if staying on in education.
